Enrollment Forms
Enroll now in Havasuonline.


Step 1: Reserve your spot, register online.

If you have not already done so, register online to save your spot in Havasuonline.

Step 2: Submit your Enrollment Application and required documents*.

Once registered online, complete and submit all required forms by fax, mail, or email. New Havasuonline students are required to submit forms and documentation prior to acceptance at our school.

New Students

New students who are planning to only attend Havasuonline full- or part-time will need to complete and submit the Enrollment Application. New students also need to submit all documents listed under Required Documents (see below) to complete enrollment.

The Enrollment Application [801 KB] includes the following forms which may be submitted separately:

  • Student Residency Questionnaire
  • Records Release
  • Home Language Survey
  • Compliant Immunization Records or Signed Immunization Waiver
Concurrent Students

Concurrent students need to complete and submit the Concurrent Enrollment Form [342 KB]. Concurrent students also need to submit all documents listed under Required Documents (see below) to complete enrollment.

Required Documents:
  • Certified Birth Certificate - Each student is required to mail a certified copy of their birth certificate within 30 days of entrance to our school or student will be removed from enrollment. If you choose to fax or email a certified copy of the birth certificate, it must contain the word "certified" officially stamped somewhere on the document.
  • Transcript*** - Each student will be required to fax, mail, or email a copy of transcript from previous school attended, including AIMS scores if the student is entering from an Arizona school. Our academic counselor will need this information to help students with required course placement.
  • Special Education (IEP & Psychological Report/Evaluation) - Students with an IEP (Individualized Education Plan) should provide a copy of their IEP and Psychological Report/Evaluation. Our Special Education Department will review and recommend placement in classes.
  • Guardianship Papers (temporary or permanent) - Students who have a temporary or permanent legal guardian will be required to provide documentation that states the guardian is able to assist in medical and educational needs. When custody arrangements exclude a parent (sole custody), we will need a copy of the court order/divorce decree stating custody arrangements.
  • Immunization Records - Each student must fax, mail or email in a copy of their immunization records showing dates of all required shots. This documentation is required before the student will be allowed to attend school. If you choose not to immunize your child (due to religious or other personal reasons), you are required to submit the Immunization Exemption Form [29 KB].

Once your Enrollment Application is processed, your academic advisor will contact you to help you select your courses.
